The 2026 PREP randomised trial compared e-cigarettes and nicotine patches in 1,140 pregnant smokers. E-cigarettes showed a modestly higher quit rate (6.8% vs 4.4%) and neither method was linked to increased adverse pregnancy outcomes — but NHS guidance still recommends licensed NRT first; vaping may be supported if it helps a woman remain smokefree.
